Chapter Officer Reporting Form

The Chapter President leaving office (or another designated officer) should submit a copy of this form immediately following the chapter's election of officers. This form may be also submitted if there is a change in chapter leadership during the year.

Chapter Presidents are encouraged to submit their pictures to be displayed with the listing of ALA Chapters and Chapter Presidents. Digital photographs, and those in jpg, tif, gif or eps format, are preferred and may be emailed to
